DO YOU HAVE A CHILD, THAT REQUIRE A SHOT RECORDS TRANSFER TO A FLORIDA FORM DH 680:

           IF YES: Please email a copy of your child's shot records to: shotrecords@back2schoolhealthclinic.org or you may upload it during registration.

          EN CASO AFIRMATIVO: Envíe por correo electrónico una copia de los registros de vacunas de su hijo a: shotrecords@back2schoolhealthclinic.org o puede cargarla durante el registro.

             IF NO: (Please review the following): Please contact your child’s health care provider who maintain your child’s immunization records. Their office can provide you with your child’s State Certification of Immunization. Hillsborough County Public Schools require the Florida Certification of Immunization (Form DH 680). Florida Form DH 680, must be used to document receipt of immunizations required for entry and attendance in Florida schools, childcare facilities, and family daycare homes.

Another place to look is at the County Health Department (CHD). If you received any immunizations there, they may be able to provide you with an immunization history. Locate a CHD in your area at www.floridahealth.gov/public-health-in-your-life/county-health-departments/CHDlisting.

If your child has attended a child care center or school, they may have a copy of your child’s immunization history on file at the center/school depending on how long ago they attended.
​
You are required to have your child's shot records for the routine immunization and/or shot records transfer appointment.

          SI NO: (Revise lo siguiente): Póngase en contacto con el proveedor de atención médica de su hijo, que mantiene los registros de inmunización de su hijo. Su oficina puede proporcionarle la Certificación de Inmunización de Florida de su hijo (Formulario DH 680). El formulario DH 680 debe utilizarse para documentar la recepción de las vacunas necesarias para la entrada y asistencia a las escuelas, guarderías y guarderías familiares de Florida.

Debe tener los registros de vacunas de su hijo para la cita de vacunación de rutina y/o la transferencia de registros de vacunas.
