ABOUT US

Project LINK, Inc is the administrative arm of the Back-to-School Coalition of Hillsborough County. The Coalition consists of medical collaboratives that include BayCare, USF Health, Tampa General Hospital, Florida Dept of Health- Hillsborough, South University, University of Tampa Nursing, Arizona College of Nursing, Florida Chapter of American Academy of Pediatrics, and Hillsborough County Public Schools health services.
Our focus is to ensure school-aged children are provided with shot records transfers, routine vaccinations and/or school physicals to meet enrollment requirements for Hillsborough County Public Schools.
Our clinics are held annually during the summer and families are required to register in advance. To be eligible to attend a Back-2-School Health Clinic, children must meet the following requirements:

A parent or legal guardian must accompany child at all times; current Immunization records are required to receive shots and/or Immunization records transfers;

A child must be uninsured, under-insured or have Medicaid coverage; No current private insurance.

If a child is only receiving a school physical, you do not need to bring his/her immunization records.

To enter Kindergarten, the child must be 5 years or older on or before September 1.
Please note, if your child attended a Hillsborough County Public or Charter School last year, he/she DOES NOT require another school physical this academic year for enrollment. Hillsborough County School District only requires 1 physical during a child’s K – 12 tenure.

If your child requires an Immunization or Immunization Record Transfer/Update, You MUST upload a copy of the immunization record during registration and bring a hard copy of his/her shot records with you, at the time of appointment. NO EXCEPTIONS!

If your child is transferring from a private school or coming from another State or Country and his/her immunization is up to date, but require Immunization records transferred to a Florida Form DH-680, you must bring his/her current shot records with you to process the transfer. You can also contact the Florida Department of Health (813) 307-8000 for this service.
In light of the post COVID-19 concerns, The Back to School Coalition of Hillsborough County is continuing its preventative measures to reduce the spread of COVID-19 during the Back-2-School Health Clinics.
To protect the health of patients and staff, we are limiting the number of Immunizations and School Physicals offered, in addition to restricting the attendance to only 1 parent or guardian to escort child(ren) receiving medical services.

The COVID-19 VACCINE WILL BE AVAILABLE FOR CHILDREN 5 TO 18 YEARS OLD.
