The Back-to-School Health Clinic is a community program that helps families prepare their children for the school year by providing required school-entry health services in one convenient location.
These clinics are designed to support K–12 students in Hillsborough County by offering services such as:
School physical exams
Immunization record review
Assistance with the Florida School Immunization Form (DH 680)
Routine immunizations (if needed and with parent consent)
The goal is to help ensure children meet school health requirements and are ready for a smooth start to the school year.
Important Notes:
Services are provided at no cost to eligible children
A $10 administrative registration fee per family is required to reserve an appointment
Appointments are required — walk-up services are not available
Parents or legal guardians of K–12 students enrolled in Hillsborough County public or charter schools can register. Registration is completed per family, and a $10 administrative fee per family applies for scheduling and processing. A parent or legal guardian must provide consent for any clinical service delivered by licensed medical partners, and must attend the appointment or complete required consent steps during registration.
The Back-2-School Health Clinics provide school-entry health services to help children meet enrollment requirements.

Yes. You need an appointment to attend the Back-2-School Health Clinic. Appointments help your family complete check-in, immunization record review, Florida DH 680 documentation support, and the school physical with licensed medical partners. A $10 administrative fee per family is paid during registration. If you arrive without an appointment, we may not be able to serve you due to limited capacity, and we will offer the next available time slot if openings exist.
Most families complete their visit within 45 to 90 minutes from check-in to completion.
To help your visit go as quickly as possible, please:
Arrive on time
Upload your child’s current immunization records during registration
Bring any school forms you have received
Your visit may take longer if:
Immunization records are not uploaded or available
School forms need to be updated
Additional review is needed by a medical professional
Uploading your child’s immunization history in advance helps reduce wait time and allows clinic staff to prepare before your arrival.
We understand that coordinating schedules can be challenging. However, due to limited space and safety considerations, we strongly recommend that only the parent or legal guardian and the child(ren) with a scheduled appointment attend the clinic.
To help keep the clinic running smoothly and protect everyone’s privacy:
Only the parent/legal guardian and the child(ren) receiving services will be allowed in clinical and documentation areas
Additional family members may be asked to remain in designated waiting areas, if space allows
