Does Your Child Need School Health Services?
The Back-2-School Health Clinics are intended for children who:

Are new to a Hillsborough County Public or Charter School; and/or
-
Need school-required immunizations to meet enrollment requirements; and/or
-
Need assistance updating or obtaining a Florida School Immunization Form (DH 680); and/or
-
Need a school physical exam required for school enrollment purposes only.
If your child is already up to date on school-required immunizations, has a current DH 680 form on file, and does not need a school physical, your child may not require services through the Back-2-School Health Clinics.
If you are unsure whether your child needs services, you may continue with registration and answer the eligibility questions provided.
Registration Fee Notice
A one-time $10 administrative registration fee is required for each family account to reserve appointments for one or more children.
This fee:

Applies once per family account, not per child.

Supports appointment scheduling, registration processing, communications, and clinic operations.

Is not a payment for medical services.

Is non-refundable.

Is required before appointments can be confirmed.
