By choosing not to agree to the terms outlined in the Volunteer Waiver, Release of Liability, and Disclaimer, you acknowledge that you are ineligible to participate as a volunteer for Project LINK’s Back-2-School Health Clinics.

For the safety and legal protection of all participants, staff, and community members, all volunteers must review and accept the waiver in full prior to engagement in any clinic-related activities.

We appreciate your interest in supporting our efforts and invite you to explore future opportunities with Project LINK where alternative participation options may be available.
